Our professional office staff strives to make scheduling appointments easy when you contact Memorial and Katy Surgical Specialists. The office is closed on weekends and most holidays. We have an answering service available after office hours. The physicians rotate call and are available 24 hours a day for emergencies by calling the office telephone number.

We accept new patients who have been evaluated by a primary care or referring physician who has recommended surgery.

New patient registration forms can be downloaded from the forms section. Please complete and bring with you to the appointment. Please arrive 30 minutes prior to your appointment time to complete the registration process. You will be asked to present your insurance card and a photo identification card to be copied for your chart.

If you have had any diagnostic testing performed, it is very important that the doctor have the reports and CD’s available at the time of the evaluation. This also includes any lab work or reports from your referring physician. Please bring all reports and CD’s with you to the appointment.

We accept most major insurance plans including Medicare and Medicaid. Please refer to your insurance company provider directory to confirm that our physicians are in your network.

We will verify your insurance prior to the appointment. All co-pays, coinsurance or deductibles will be explained to you prior to your visit.

Patients who have selected an HMO plan are required to have a referral from the primary care physician prior to the visit. Please contact your primary care physician to obtain the referral and bring it with you to the appointment.

Surgery: What to Expect

The initial office evaluation will determine if you are a candidate for surgery. If surgery is indicated, our surgery scheduler will contact you within three days after the evaluation to discuss dates, times, facilities, and pre-operative preparation.

Some surgical cases may require an assistant surgeon and/or surgical assistant. Such assistants are contracted by the surgical facility and bill independently under their group name. Questions regarding fees and insurance benefits for the surgical assistant should be directed to the billing office of that group. Some ancillary groups (pathology, radiology, anesthesiology, etc) providing services may not be contracted with all insurance. Please consult your insurance carrier to determine in network benefits.

A billing representative from our office will contact you after the evaluation to discuss your insurance benefits for the surgeon’s fee. Your estimated financial obligation will be required as a surgical deposit prior to the procedure.
